Not yet managed to get to a repeatable case to create a ticket, but it happens a lot. Upload an image - use it - all works fine.
Later when in the upload library spot that there are two copies of the image. (But which one is the post using? So you leave them both.)

@JohnPhilpin Noted. Any chance you are using the Mac app to upload? There was a bug very similar to this that I fixed just a week or two ago.

Whilst there I updated and then set about replying including the upload above.
Only one image in the library. (I believe a ‘w00t’ is in order.)
The problem might well be solved.
