The word refuses suggests that someone did - but they are private individuals.

@JohnPhilpin This is a follow-up to what happened when they moved to Canada. There was protest from Canadians who didn't like the idea of paying for their security there, so they agreed to pay it themselves. So now they will be paying in the US. For some reason the tabloids consider even the slightest details about their lives are worthy of publication. Silly.
