When I enter a patient’s room with COVID, I nearly roll my eyes at the requirement to wear an N-95 mask and goggles.  As if it’s significantly different now (in my young patient population) than another coronavirus infection (just regular mask for face covering). Or, just empowering people to say, there’s good data that says “x” doesn’t improve what everyone says it does.  And get buy-in to stop collecting extraneous, potentially harmful data.</p>\n<p>At the macro level, increasingly, healthcare spending isn’t on patient care or even physician/staff salaries, which is a whole other challenge. I don’t work in obstetrics, but do sometimes work with very young patients.  I think some fields of medicine are definitely practiced with the threat of lawsuit in the background.  And physicians absolutely like more information, despite documentation (as referenced in the article) that more information can cause harm.  In infants/young children with viral bronchiolitis, there’s lots of evidence that continuous pulse oximetry prolongs hospitalization without improving any outcomes.  Some places I’ve worked are devoted to this continuous vital sign, more out of habit than thoughtfulness.  Unfortunately. I have an affection for the idea of faith; perhaps because I grew up in a church named Sola Fide.  It was a strange name for visitors, so the pastors over the years often talked of faith, which I took to mean, rather than “belief without proof,” but trust beyond understanding.  That’s probably not terribly different than Anna’s thoughts.</p>\n<p>Growing up in that church (in a confessional Lutheran tradition), part of Christianity was the mystery.  So many things don’t quite make sense, can’t make sense to us.  There’s always questions, and things that don’t make sense, theologically and the world in whcih we live, so we practice and have faith and trust, even when that complete understanding eludes us.</p>\n<p>I think of science in my day job in much the same way.  A lecturer today referenced the phrase, “trust the science,” aptly noting that phase means nothing.  Science is the pursuit of understanding, and so much of what we understand now will prove to be wrong as new tools/techniques become available, and what we thought was simple is far more complex (or maybe wrong).  Make claims using the scientific method with humility, because some of those claims will be inaccurate.  Doesn’t mean we shouldn’t try to learn more or to advocate for best practices based on current knowledge.  It’s a practice toward the physical world in much the way that religion is a practice for the physical and spiritual world. Our eldest (my step) is in the process of transferring schools because there’s one intro class she needs, and she just needs a different environment for her to get through it.  Her dad would rather she change to a major that doesn’t require the class, because, to him, the degree is the point.  College, graduate work, all of it, is a path to an endpoint.  Sometimes you know it, sometimes you don’t.  Sometimes there’s one way to get to a point (I finished 27th grade last year to get to where I wanted to be), but, sometimes, there’s so many paths to get somewhere.  We keep emphasizing to all the kids that, even if you know where you think you want to go, any path there (and associated detours) can teach you so much if you’re paying attention. The Mac version arrived in the mail about a week ago, and I promptly connected it to my office Windows machine.  My limited mechanical keyboard experience is mostly low profile Gateron Reds, so the Browns are pretty similar, slightly clickier.  The default for the keyboard is, when you press their equivalent of the function key, it locks the F1-F12 buttons into that mode, as well as the intergrated number pad (on the right hand keys), as well as the arrow keys.  These stay functional until you press the function key again.  This works will in my polysomnography software but is less convenient tapping Fn-F2 to jump to different points of an Electronic Medical Record document to fill it out.  Still haven’t typed enough to have a handle on the different shapes/positions of some keys.  I have learned that I make more mistakes with negative tilt than with the keyboard in a more neutral position.
